3. ALL WORK RELATED INJURY and INCIDENT NOTIFICATION
should be made to the INCIDENT NOTIFICATION HOTLINE as soon as possible.

When a staff member is involved in an INCIDENT or is INJURED AND /OR SEEKING COMPENSATION, THEY SHOULD:
1. Seek FIRST AID OR MEDICAL ATTENTION from a doctor.
Obtain a Workcover Doctor’s Certificate. If you do need to take time off work you will need a
FINAL WORKCOVER DOCTOR’S CERTIFICATE before returning to work.
2. NOTIFY your immediate supervisor.
3. RING FREECALL 1800 811 523 7am to 7 pm Monday to Friday.
If you cannot call yourself, somebody else can call on your behalf with the relevant details.
